Case-Management Services

assists the clients and their families with completing referrals to financial, social, medical, mental health and other appropriate providers so that they may improve their ability to function in the community. Services include, but are not limited to:
assessments,
individualized planning,
monitoring of services
advocacy.

Services are provided in the home and community.

The VA's " Aid & Attendance" Benfits is for watime veterans or their surviving spouses, who meet certain medical and Finaiancial Requirements

Hope HomeCare is partner with the VETAssist Program of VETERAN HOMECARE

HOPE HOMECARE :

coordinator will assist with the referral process, linkage you to the Veteran Department at the Vet Assist in obtaining your Aid and Attendance benfits
